Agree that I do not have any medical condition that would prevent me from participating in intense physical exercise and will notify coaches of any condition that may impact my ability to participate in strenuous physical activity.

Agree that in participating in the training I do so on the distinct understanding that I do so entirely at my own risk. I am aware accidents or damage to property or injury or even death is possible while undertaking training.

Agree to abide by the Cabra Kai Academy code of conduct rules and acknowledge that any failure to abide by rules or negative behaviour may result in refusal of training and/or cancellation of membership and possibly being banned from further training. 

Agree to acknowledge that because of the risks involved to release to the full extent permitted by law all claims resulting from any accident or damage to property or injury or death while undertaking training.

Accept and agree to the terms and conditions outlined in this online waiver and fully understand and acknowledge that martial arts training is done entirely at my own risk.
